Our beneficiaries have multiple options about where to get the COVID-19 vaccine. We've provided this QR Code for you to place on your print and digital product to make it simple for our beneficiaries to take out their phone and scan this code to check the Vaccine Appointments page!

This single location features the DOD vaccination sites. Patients can scan to find a local military hospital or clinic or local/state vaccine site and how to get an appointment.

To add this to your MTF website, search for the image in the AFPIMS Shared Media Manager: [210415-A-AB123-002]
